BEHAVIORAL HEALTH SERVICES CPLC offers outpatient behavioral health services to families, adults, children, and adolescents; shelter and services for women and children who are victims of domestic violence, HIV behavioral health treatment and pre-natal care to South Phoenix women.CENTRO DE LA FAMILIA & VIA DE AMISTADThe program provides outpatient behavioral health services to a myriad of families, adults, children, and adolescents. Clients receive the following services: psychiatric evaluations, medication management, counseling services, marriage and family counseling, group counseling and case management for children and adults. Via also provides individual counseling. Specific substance abuse groups are offered in both English and Spanish up to three (3) times a week. A total of one thousand six hundred (1,600) clients utilized the services.CORAZONCorazon's purpose is to deliver Adult Residential Alcohol/Drug Abuse Counseling Treatment Services to individuals and families in the community. The services include assistance in the self-administration of medication and counseling services in the form of individual, group, family counseling, and case management services. Transitional living and aftercare services are available. Supported employment services are provided in the form of pre-job training/educational and development, job coaching and employment supports. Specific substance abuse groups are offered in English and Spanish. Three hundred fifty (350) clients received services.RYAN WHITE CARE SERVICESThe program has two basic components, Behavioral Health Treatment and Targeted Outreach to the Latino community. The vast majority of the clients that we provide services to are Latino, individuals who are predominantly Spanish-speaking and who are uninsured or underinsured. The behavioral health component provided services both in group and individual sessions.DE COLORESA program dedicated to women and children of domestic violence by providing shelter and services that include: twenty-four (24) hour information and referral hot line, shelter and meals, individual and group counseling, case management, legal advocacy, COMADRES follow-up program, computer and life building skills, homework assistance, and the Ahijado program (for children three (3) years and older) to enhance the journey of healing, to women and children, who have suffered from Domestic Violence. They learn about the cycle of violence through an assessment process, are able to work on issues that enhance their lifestyles. The primary focus is the monolingual Spanish-speaking women and children. In 2004, three hundred thirty nine (339) women and children were provided shelter and counseling.

ECONOMIC DEVELOPMENT The foundation of any vital and healthy community is the well being of its residents. CPLC pursues the development of services key to the welfare of its constituents. Through its economic development component, CPLC offers workforce development services for youth and adults. CPLC provides a summer youth employment program. In addition CPLC creates and maintains retail and commercial properties aimed at creating jobs, eliminating blight, promoting small business development, and improving the infrastructure of the south Phoenix communities.WESTSIDE SUMMER YOUTH PROGRAMCPLC's Westside Workforce Development Center (WWDC) in partnership with the City of Phoenix, provide city youth between the ages of fourteen (14) through twenty-one (21) in school and out-of-school the prospect to obtain job exposure in various professions, paid hands on work experience plus the opportunity to experience for themselves the importance of an education. In 2004, two hundred fifty (275) youth participated in the summer youth program.WESTSIDE WORKFORCE DEVELOPMENT CENTERUnder the Workforce Investment Act (WIA) Adult Program, the West Side Workforce Development Center (WWDC) works with individuals who are eighteen (18) years of age and older. WWDC serves people in need of various services including English as a Second Language (ESL), General Adult Education classes (GED), occupational training, job placement assistance and support services. In 2004, nine hundred eleven (911) individuals received employment and training services.YOUTH OFFENDERS PROGRAMUnder the umbrella of the CPLC West Side Workforce Development Center, the Youth Offender Program services individuals between the ages of sixteen (16) to twenty-one (21), who are economically disadvantaged, who have had involvement with the Judicial System and have dropped out of school. The program offers a kaleidoscope of services that include: work readiness skills, survival of daily living skills, mentoring, and guidance. Five Hundred, Fifty Nine (559) youth utilized the services and participated in the program during 2004.PRESTAMOS (A SUBSIDIARY OF CPLC)The program provides economic opportunities for small and emerging small businesses in Arizona, by providing loans along with technical assistance. The following loan programs are offered: SBA Microloan Program addresses the problems encountered by small business entrepreneurs seeking financing within the range of $2,000 to $35,000, and emerging micro-enterprises within the target areas. The SBA Community Express Loan provides financing for business needs that fall between the Microloan program and conventional loan programs. Loans range from $5,000 to $75,000. In 2004, Prestamos granted twenty-three (23) loans totaling five hundred sixty thousand dollars ($560,000).SMALL BUSINESS LOAN PROGRAMThe program offers loans to small and minority businesses for the purpose of meeting the national objective of job creation and business expansion. CPLC provided ninety thousand doiiars ($90,000) in loan funds.

EDUCATION Education is a critical investment in our future. Education is teaching, guiding, supporting, encouraging and acknowledging all children, youth and adults, regardless of their economic and social status, ethnicity, creed, age, and gender. Education can be integrated classroom instruction through traditional and nontraditional education systems; or it may merge parenting skills training, school enrichment programs and community involvement. CPLC's goal is to afford every child and adult the opportunity to achieve a quality education.EARLY HEAD STARTThis is a federally-funded, community-based program for low-income pregnant women, infants, toddlers, and their families. The focus of this program is to promote healthy prenatal outcomes for pregnant women, enhance the development of very young children, and promote healthy family functioning. During 2004, the programs served 63 infants and toddlers.ARIZONA MIGRANT & SEASONAL HEAD STARTThis is a federally-funded, early childhood education program that prepares children from disadvantaged households for their first years of schooling. Children develop cognitive and social skills in a rich learning environment. Not only does the program provide full-day childcare and preschool services, it serves as a gateway through which some of Arizona's most underserved families can access a wide range of health and human services, such as citizenship, GED and ESL classes, housing assistance, legal aid, immunizations, nutrition information, income tax preparation, and even food, clothing, financial aid, and other forms of emergency assistance. During 2004, the program served six hundred eighty-five (685) children.

HOUSING Affordable quality housing has always been at the forefront of CPLC's comprehensive service system. CPLC's housing programs offer counseling services to clients for mortgage default, tenant/landlord issues, pre and post purchase and fair housing. We develop and build affordable single-family housing for low-income families in economically distressed neighborhoods and rehabilitate homes under federally, funded programs.HOUSING COUNSELING & EMERGENCY ASSISTANCEThe program provides mortgage default and rental delinquency counseling for low to moderate income residents. During 2004, two hundred forty four (244) households received counseling services.HOME PURCHASING and CLOSING COST ASSISTANCECPLC provides homebuyer education, offers workshops and individual counseling. During 2004, four hundred seventy six (476) households received homebuyer education.SENIOR HOUSINGIn order to address the needs of Arizona's growing senior population, a comprehensive, center based program that offers advocacy and case management services, which include but are not limed to medical checkups, safe transportation, recreation, entertainment, and companionship. 7,406 senior adults and handicapped individuals were provided congregate and home delivered meals. CPLC has a total of three hundred fifty-three (353) senior housing units.CPLC's NOGALES SELF-HELP HOUSING PROGRAMThis program provides opportunities for families in rural Arizona to cash in on ""sweat equity"" by providing their own labor. Families participating in the self-help homeownership program are able to defray housing costs, become more self-sufficient, and develop pride in their homes and communities.SOMERTON IMMIGRATION PROGRAMCertified staff assisted families in securing citizenship and legal residence in the United States and assisted individuals with getting their immigration cards. Clients are represented in immigration courts and all required immigration forms are completed for the adults and children. In addition, there is a Notary Public on site to notarize and translate for clients. During 2004, three thousand two hundred ninety (3,290) clients received the following services: citizenship classes, GED, English as a second language, counseling, information and referral, and adult basic education classes.AFFORDABLE HOUSINGProvides affordable housing to populations including the elderly, handicapped or disabled. CPLC has 3,665 units in various properties located throughout the state of Arizona.

To Expand The Opportunities Available To Said Residents And Groups To Own, Manage, And Operate Business Enterprises By Furthering The Development Of Locally Owned Or Locally Operated Business Enterprises In Economically Underprivileged Or Depressed Areas; To Assist Said Residents And Groups In Receiving Entrepreneurial And Management Skills Necessary For The Successful Operation Of Business Enterprises; To Provide Financial Support For The Successful Operation Of Business Enterprises By Said Residents And Groups; To Assist Said Residents & Groups In Obtaining Such Financial Support From Other Sources. To Expand Opportunities Available To Said Residents And Groups To Obtain Adequate Low-Cost Housing Accommodations. To Aid, Support And Assist By Gifts, Contributions Or Otherwise, Other Corporations, Community Chests, Funds, And Foundations Organized And Operated Exclusively For Purposes, No Part Of The Net Earnings Of Which Inures To The Benefit Of Any Private Shareholder Or Individual, And No Substantial Part Of The Activities Of Which Is Carrying On Of Propaganda, Or Otherwise Attempting To Influence Legislation. To Provide Elderly Persons And Handicapped Persons With Housing Facilities And Services Specially Designed To Meet Their Physical, Social, And Psychological Needs, And To Promote Their Health, Security, Happiness, And Usefulness In Longer Living, The Charges For Such Facilities And Services To Be Predicated Upon The Provision, Maintenance And Operation Thereof On A Nonprofit Basis.

YOUTH COMMUNITIES BASED YOUTH PROGRAMS Empowers youth to lead healthy successful lifestyles by maximizing their educational attainment, increasing their resiliency and engaging in activities that reflect family/cultural values and community pride. The following services are also available: mentoring, life-skills groups, academic assistance, parental outreach/engagement, and alternative activities.MOTHERS AGAINST GANGS (MAG)MAG provides young adults and their families with alternatives to destructive behavior through constructive activities that lead to new life choices. During 2004, MAG provided youth and families with a safe and nurturing environment, successfully facilitating positive peer interaction through drop-in center activities.CARL HAYDEN YOUTH CENTERThe youth center provided educational and recreational programs to seven hundred fifty (750) residents of West Phoenix. CHYC provides youth with a safe and nurturing environment, successfully facilitating positive peer interaction through drop-in center activities. The Youth Empowerment Program has been enhanced by the ARCO Project that includes mural making, teatro, and creative writing that will include a web based magazine which was implemented into the center in 2004, providing academic and life skills development training to youth and their families.YOUTH DROP IN CENTER (RESOURCE FACILITY IN TUCSON FOR INNER-CITY YOUTH)The Brazos Abiertos program offers students academic assistance and tutoring. Computers are made available to youth on-site for completing school class work and projects. The Youth Drop-In Center provides case management services and implements activities that emphasize cultural and intergenerational awareness.SOMERTON AFTER SCHOOL COMPUTER LABAn after school computer lab for the youth of Somerton is provided to facilitate the completion of homework. A tutor is on-site to help the youth with any computer problems. The computer lab use is, free and available to middle school, high school, and college students. Four hundred twenty eight (428) students participated in the program.

RYAN WHITE CASE SERVICES- (PART OF CENTRO) THE PROGRAM HAS TWO BASIC COMPONENTS, BEHAVIORAL HEALTH TREATMENT AND TARGETED OUTREACH TO THE LATINO COMMUNITY. THE CAST MAJORITY OF THE CLIENTS THAT WE PROVIDE SERVIES TO ARE LATINO, INDIVIDUALS WHO ARE PREDOMINANTLY SPANISH-SPEAKING AND WHO ARE UNINSURED OR UNDERINSURED. CORAZON IS A LICENSED LEVEL II RESIDENTIAL SUBSTANCE ABUSE TREATMENT CENTER FOR MEN OVER THE AGE OF EIGHTEEN. CORAZON HAS BEEN PROVIDING SERVICES TO THE COMMUNITY SINCE 1983. THE CENTER CURRENTLY HAS 41 BEDS FOR IN PATIENT SERVICES AND 17 BEDS FOR THE SIX MONTH TRANSITIONAL LIVING PROGRAM. THE CENTER SPECIALIZES IN PROVIDING SUBSTANCE ABUSE TREATMENT IN AN ENVIRONMENT THAT IS CULTURALLY SENSITIVE AND INCLUSIVE. CORAZON UTILIZES A VARIETY OF TREATMENT MODALITIES, INTEGRATING IDENTIFIED BEST PRACTICES WITH TRADITIONAL HEALING ACTIVITIES. CORAZON PROVIDES THE TOOLS NECESSARY FOR THE MEN TO BE SUCCESSFUL AS THEY PURSUE A LIFE LONG CHALLENGE TO LIVE FREE OF SUBSTANCE ABUSE. BETWEEN BOTH PROGRAMS APPROXIMATELY 250 CLIENTS ARE SERVED ANNUALLY BY CORAZON PROGRAMS.

THE PROGRAM UTILIZES RESOURCES TO ATTRACK CAPITAL AND INCREASE PHYSICAL, COMMRECIAL, AND BUSINESS DEVELOPMENT THEREFORE CREATING EDUCATION AND JOB OPPORTUNITIES FOR THE RESIDENTS OF THE NEIGHBORHOOD BEING SERVED. UNDER THIS DIVISION VARIOUS MISSION DRIVEN DEVELOPMENT ACTIITIES OCCUR INCLUDING: MULTI-FAMILY, COMMRCIAL AND SINGLE FAMILY CONSTRUCTION, SINGLE FAMILY SELF-HELP HOUSING AND ACTING AS A GENERAL CONTRACTOR ON PROJECTS.

THE SHELTER WAS OPENED IN 1986 WITH 16 BEDS. TODAY DE COLORED HAS 58 BEDS FOR THE CRISIS PROGRAM AND 42 BEDS FOR THE TRANSITIONAL LIVING PROGRAM. THE STAFF IS BI-CULTURAL/BI-LINUAL AND SPECIALIZES IN SERVING MONOLINGUAL SPANISH SPEAKING WOMEN. THE PROGRAM PROVIDES ALL OF THE BASIC NEEDS FOR THE FAMILIES LIVING IN THE CRISIS PROGRAM. FOR THE FAMILIES LIVING IN THE TRANSITIONAL PROGRAM THE SHELTER PROVIDES APARTMENTS AND TRAINING THAT WILL ASSIST THEM AS THEY BEGIN THEIR JOURNEY TOWARD HEALING AND INDEPENDENCE. THE SHELTER SERVES APPROXIMATLEY 430 CLIENTS ANNUALLY WITH MAJORITY BEING CHILDREN. THE AVERAGE NUMBER OF CHILDREN ENTERING WITH THEIR MOTHER IS FOUR. THE SHELTER PROVIDES SERVICES FOR BOTH THE WOMEN AND CHILDREN THAT HAVE BEEN CREATED TO BE RELEVANT FOR THE LATINO POPULATION. THE CENTER OFFERS SERVICES THAT REVOLVE AROUND EDUCATION, EMPLOYMENT, AND TRAINING.ARIZONA MIGRANT & SEASONAL HEAD START:THIS IS A FEDERALLY-FUNDED, EARLY CHILDHOOD EDUCATION PROGRAM THAT ISAVAIABLE TO MIGRANT AND SEASONAL FAMILIES AND SERVES CHILDREN FROM 6 WEEKS OLD TO SIX YEARS OLD. THE PROGRAM IS INDIVIDUAIZED, MULTI-CULTURAL AND UTLIZES APPROPRIATE DEVELOPMENTAL PRACTICES. CHILDREN LEARN TO BE SELF DIRECTED AND INTERACT IN GROUP SETTINGS. STAFF DEVELOP PARTNERSHIPS WITH PARENTS TO INVOLVE THEM AS THE PRIME EDUCATOR IN THE DEVELOPMENT OF THEIR CHILDREN. THE PROGRAM SERVES OVER 900 CHILDREN AND FAMILES ANNUALLY.EARLY HEADSTART: THIS IS A FEDERALLY-FUNDED, COMMUNITY-BASED PROGRAM FOR LOW-INCOME PREGNANT WOMEN, INFANTS, TODDLERS AND THEIR FAMILIES. MULTIFAMILY APARTMENTS:PROVIING AFFORDABLE RENTAL UNITS HAS BEEN A MAJOR COMPONENT OF CPLCS AFFORADABLE HOUSING EFFORTS.
