CONSUMER DIRECTED PERSONAL ASSISTANCE PROGRAM (CDPAP)- TO PERMIT CHRONICALLY ILL AND/OR PHYSICALLY DISABLED INDIVIDUALS GREATER FLEXIBILITY AND FREEDIOM OF CHOICE IN OBTAINING HOME CARE SERVICE. SERVING APPROXIMATELY 501 CLIENTS.

Home Attendant/Housekeeper Program - to provide personal care services in the home of NYC patients who are medically disabled, aged and/or physically handicapped who might otherwise require institutionalization. Serving approximately from 1,333 clients at 7/1/12 to 256 clients at 6/30/13. (The program reported here includes the programs of Manhattan, Brooklyn, and the Bronx reported on Part III, line 4b, 4c, and 4d, Form 990 - 2012.)
